Folate Deficiency: Why the Form Changes the Number
Folate deficiency: why the form changes the number

Isolated folate deficiency is uncommon. When it shows up, it usually arrives with a poor diet, alcohol use, or a malabsorption problem, and the main clinical sign is megaloblastic anemia, the same anemia seen in vitamin B12 deficiency. Adult RDAs are 400 mcg DFE. Folic acid from a supplement counts for more DFE per microgram than folate in food, because more of it is absorbed.
What is folate deficiency, in plain terms?
The NIH Office of Dietary Supplements says isolated folate deficiency is uncommon. It usually coexists with other nutrient shortfalls, because it tracks with poor diet, alcoholism, and disorders that impair absorption.
The primary clinical sign of folate deficiency or vitamin B12 deficiency is megaloblastic anemia: large red blood cells with abnormal nuclei. That sign does not tell you which vitamin is low. A tired afternoon does not either. Frank deficiency is described as rare in the United States, with some people still sitting at a marginal status.
Those lines are from the NIH Office of Dietary Supplements folate fact sheet, updated November 30, 2022.
How much folate do adults need?
The adult RDA is 400 mcg DFE a day for men and women 19 and older. The unit is dietary folate equivalents, not raw micrograms of whatever is in the bottle.
The Food and Nutrition Board built DFE because folic acid is absorbed more readily than food folate. At least 85 percent of folic acid is estimated to be bioavailable when taken with food. About 50 percent of folate naturally present in food is bioavailable. From that, the board set three conversions.
| 1 mcg DFE equals | Which form |
|---|---|
| 1 mcg | Food folate |
| 0.6 mcg | Folic acid from fortified food or a supplement taken with food |
| 0.5 mcg | Folic acid from a supplement taken on an empty stomach |
Conversions from the NIH folate fact sheet. Factors for converting 5-MTHF into mcg DFE have not been formally established.
Are folate, folic acid, and 5-MTHF the same thing?
Folate is the generic name. It covers naturally occurring food folates and the forms used in supplements and fortified foods, including folic acid. Food folates are tetrahydrofolate forms, usually with extra glutamate residues.
Folic acid is the fully oxidized monoglutamate form used in fortified foods and in most supplements. Some supplements use 5-MTHF instead, also called L-5-MTHF, 5-methyl-folate, L-methylfolate, or methylfolate. The fact sheet is explicit that conversion factors from mcg of 5-MTHF to mcg DFE have not been formally established. A label that prints both numbers is doing math the Food and Nutrition Board has not locked in.
Who is more likely to run low?
People with alcohol use disorder are the group the fact sheet leads with. Diets in that group are often low in folate, and alcohol interferes with absorption and liver uptake, speeds breakdown, and increases loss in urine.
Marginal status is not the same as the anemia. Frank deficiency is rare in the United States. A clinician reading a blood count is the person who can tell those apart. This page cannot.
Why does a high folate intake worry B12 researchers?
Large amounts of folate can correct the megaloblastic anemia of vitamin B12 deficiency without correcting the neurological damage. Some experts have worried that high supplement intakes could hide a B12 problem until that damage is hard to undo.
The fact sheet says questions remain, and that concern has shifted toward whether large amounts of folate can precipitate or worsen the anemia and cognitive symptoms tied to B12 deficiency. The upper level applies to synthetic forms from supplements and fortified foods, not to folate from food. The board did not set an upper level for food folate, because high intakes from food have not been reported to cause adverse effects.
Where does a supplement fit?
Food folate and fortified foods are the intake the RDA was built around. A supplement is an added source, not a blood test.
Methyl Multi is made with active, body-ready forms. This page does not list its folate form, its micrograms, or any test result. If a clinician is already following a low count, follow that plan.
Questions people ask next
Is 400 mcg on a label the same as 400 mcg DFE?
Only if the form is food folate. Folic acid taken with food converts at 0.6 mcg per 1 mcg DFE. Taken on an empty stomach, the factor is 0.5 mcg. Read the unit on the label before comparing it with the RDA.
Does megaloblastic anemia mean folate is the missing vitamin?
No. The fact sheet names it as the primary clinical sign of folate deficiency or vitamin B12 deficiency. The blood picture alone does not separate the two.
Has a DFE factor been set for 5-MTHF?
Not formally. The fact sheet says conversion factors for supplemental 5-MTHF have not been established. That is a labeling gap, not a reason to invent a multiplier.
Does the upper level include spinach?
No. The board set no upper level for folate from food. The ceiling applies to folic acid and other synthetic forms in supplements and fortified foods.
